Teams discuss for a long time, collect arguments and appear aligned. But at the end it remains unclear who really decides, which option was chosen, what follows from it and who owns the outcome.
Agreement is confused with decision.
Why this is dangerous
Apparent consensus consumes time. Decisions are postponed, responsibility is distributed but not owned, and projects continue although no real decision has been made.
